The Michiana Air Activities Flying Club (MAA) is a nonprofit group of pilots who share ownership and access to club aircraft based in Elkhart, Indiana. Membership Application

Use this form to apply for membership in the Michiana Air Activities Flying Club and provide the information needed to begin the onboarding process. Submit the completed application as instructed so the club can review and follow up with next steps.

MAA By-Laws

Pilot Qualifications & Signoffs

Aircraft Checkout Request and Signoff

This form documents your aircraft checkout and any required training to operate a club aircraft. It confirms you’ve met the club’s standards and have been approved to fly that specific aircraft.

CFI Aircraft Instruction Signoff

This document is used by a Certified Flight Instructor to verify training, endorsements, or checkout completion as required by the club. It provides a clear record that instructional requirements were met and acknowledged.

Proficiency Check

Use this form to document periodic proficiency checks required by the club to help maintain safe, current flying. It confirms completion of the check and notes any follow-up training recommendations.

Pilot History Form

This form captures your flying background, experience, and recent currency for club records. It helps the club and instructors tailor checkouts and ensure appropriate, safe access to aircraft.

MAA Waivers

Adult Waiver

This waiver is required for adult participants and acknowledges risks associated with aviation activities and club operations. It helps ensure everyone understands responsibilities and liability considerations before participating.

Minor Waiver

This waiver is required for minors and must be completed by a parent or legal guardian. It grants permission for participation and documents acknowledgement of risks and liability considerations.
